Subject: Formal Dispute of Inaccurate Account Reporting

To Whom It May Concern:

I am writing to formally dispute the inaccurate and unverifiable reporting of the following account(s) listed on my credit report. These accounts are being reported as charged-off, delinquent, or in collection status, each with an outstanding balance. This information is both inaccurate and legally non-compliant with the standards set forth by federal law.

  1. Creditor Name: [Insert Creditor #1] Account Ending In: [XXXX] Reported Balance: $[Amount]
  2. Creditor Name: [Insert Creditor #2] Account Ending In: [XXXX] Reported Balance: $[Amount]
  3. Creditor Name: [Insert Creditor #3] Account Ending In: [XXXX] Reported Balance: $[Amount]

(Add or remove account entries as needed)

Under the Fair Credit Reporting Act (FCRA) and 15 U.S. Code § 6802 of the Gramm Leach Bliley Act, financial institutions are strictly prohibited from disclosing a consumer’s nonpublic personal information without proper disclosure, authorization, and safeguards. I have never been provided such disclosure nor given consent for the sharing and reporting of this information to a third-party credit bureau in a manner that results in reputational and financial harm.

Additionally, under 15 U.S.C. § 1681i, you are required to investigate and delete any information that is inaccurate, unverifiable, or outdated. I am requesting the immediate deletion of these accounts for the following reasons:

  • Lack of verification – The furnishers have failed to provide any original documentation bearing my signature that validates these debts.
  • Violation of my privacy rights under 15 U.S.C. § 6802, as there has been no documented consent from me to disclose or share this information in the manner reported.
  • These accounts are listed as charged-off, collection or otherwise derogatory, yet no legal validation or agreement has been provided showing I am contractually bound to the balances or status reported.

I am demanding that each of these accounts be immediately removed from my credit report due to lack of verification and non-compliance with federal privacy laws. If this request is not honored, I am prepared to escalate the matter and file formal complaints with the Consumer Financial Protection Bureau (CFPB), Federal Trade Commission (FTC), and consider legal action for damages due to reputational harm and negligent reporting.

You have 30 days to complete your investigation and respond with proof of the following for each disputed account:

  1. A copy of the original signed contract from the reporting creditor.
  2. Verification that the account belongs to me.
  3. Written evidence of legal authority to report and share this data under 15 U.S.C. § 6802.

If you cannot provide this, you are required to delete each account immediately under FCRA guidelines.

Sincerely, [Your Full Name]
